Dean Yan Hua and Professor Song Dan's team break through the barriers of complex coronary artery calcification - skillfully demonstrating the PCI China solution
A gathering of heroes ignites a prairie fire. On November 23, 2024, the Beijing Cihua Medical Development Foundation and Wuhan Yaxin Hospital jointly held the Wuhan Yaxin Hospital International Surgical Live Conference. This conference was conducted in various forms, including academic lectures, on-site discussions, and surgical broadcasts, in a comprehensive, in-depth, and multi-dimensional manner. With a spirit of inclusiveness, innovation, and seeking common ground while reserving differences, it actively attracted many well-known experts, scholars, and outstanding talents to gather together. The conference specially invited Professor Yan Hua, Professor Song Dan, Professor Xu Chengyi, and Professor Wang Wei's team from Wuhan Asian Heart Hospital to complete a highly difficult and high-risk complex percutaneous coronary intervention (PCI) surgery challenge online and achieved complete success.
The patient is a 52 year old male with three vessel lesions in the left main coronary artery (LM). The right coronary artery (RCA) has diffuse lesions, while the left anterior descending artery (LAD) and circumflex artery (LCX) have vessel opening lesions, severe stenosis, and severe calcification. The patient underwent RCA intervention treatment at a local hospital 2 weeks before admission, with 2 drug-eluting stents (DES) implanted. This PCI surgery was performed with the assistance of VA-ECMO and guided by optical coherence tomography (OCT). Due to the severe calcification and multiple circular calcifications in both LAD and LCX lesions detected by OCT examination, with multiple calcified plaques thicker than 2mm, coronary artery rotational atherectomy (RA) combined with intracoronary lithotripsy (IVL) pre-treatment was first performed. Subsequently, after sufficient communication and exchange with on-site experts, Professor Yan Hua's team comprehensively evaluated the patient's calcification pretreatment situation and developed a Provisional double stent intervention treatment strategy for the patient's anterior trigeminal lesion. During the operation, drug coated balloon (DCB) PTCA treatment was performed on the proximal lesions of LCX. The entire surgical process went smoothly, and the surgical team overcame the layers of barriers of severe calcified lesions, ultimately achieving surgical success. The postoperative OCT evaluation showed satisfactory interventional treatment effects.
